By submitting this form, I acknowledge that I am voluntarily participating as a volunteer for Cary Main Street Fest, a joint event hosted by the Cary-Grove Area Chamber of Commerce and the Village of Cary. I understand and agree to the following:
1. Assumption of Risk:
I acknowledge that participation in this event involves physical activity and the potential for unforeseen risks, including but not limited to injury, illness, or accidents. I voluntarily assume all such risks and release the Cary-Grove Area Chamber of Commerce, the Village of Cary, their employees, board members, volunteers, and agents from any and all liability or claims arising from my participation.
2. Medical Treatment:
In the event of an emergency, I authorize event organizers to obtain necessary medical treatment for me (or my child, if under 18). I understand that I am responsible for any medical expenses incurred.
3. Conduct & Safety:
I agree to conduct myself responsibly and to follow all instructions given by event organizers. I understand that failure to do so may result in my dismissal from volunteer duties.
4. Photo/Media Release:
I grant permission for any photos or videos taken of me during the event to be used by the Cary-Grove Area Chamber of Commerce and the Village of Cary for promotional purposes, including social media, websites, printed materials, and advertising. I understand that these images are the property of the event organizers and that I will not receive compensation for their use.
5. Age Requirement:
I certify that I am at least 18 years of age, or if under 18, that my parent or legal guardian has reviewed and agreed to the terms of this waiver on my behalf.
6. Voluntary Participation:
I understand that my participation is voluntary and that I will not receive any compensation, benefits, or insurance coverage for volunteering.
